Patients/clients who are minors (17 years and younger) cannot be seen without a signed consent from their parent or legal guardian (with court issued documentation). A Student Wellness Center Consent for Treatment must be signed by the parent/guardian and notarized or witnessed by a Student Wellness Center staff member before or on the first visit.

Parents/Guardians may arrange to provide a consent so that it is on file before the minor needs services.

These are the following options for a parent/guardian to provide this authorization to best accommodate your needs and schedule:

Option 1: Accompany Minor to Their SWC Appointment

Bring a government issued I.D. and let the Front Desk know that you need to sign a Minor Consent for Treatment form that must be witnessed by SWC staff.

Please plan to arrive at least 30 minutes before the appointment for this. This is in addition to forms that must be filled out for the appointment.

Option 2: Download Form to Sign In Front of Notary

Download the form to print out:

Take it to a notary as a witness to you signing the form in front of them.

Submit the appropriate form for Minors consent from the Forms page.

Option 3: Meet with SWC Staff via the Internet

This process will start with an appointment via the televideo tool WebEx. The SWC staff person will validate the parent/legal guardian’s identity with a photo I.D. A picture will be taken for our records. Next, the parent/legal guardian will be provided information to log into the UNLV SWC WellnessView Patient Portal so that they may fill out the required forms for granting consent.

  3. Government I.D. to present for validation
