Nevada law allows a sole proprietor to reject workers' compensation coverage if he/she does not use employees or subcontractors in the performance of his/her contract with NSHE. See Nev. Rev. Stat. §616B.627 and 617.210.

This rejection also applies to Officers of a company. See Nev. Rev. Stat. §616B.627 and 617.207.

If a sole proprietor or officer has rejected workers' compensation coverage under applicable Nevada law, he/she must indicate the basis for the rejection of coverage and complete, sign and have notarized an Affidavit of Rejection of Coverage. The affidavit must be completed, signed and notarized prior to performance of any services under the contract with NSHE.

An unsworn declaration can be used in lieu of an affidavit if a notary is not available.
