Guidelines for Submitting Documents
Before uploading documents in the Self-Service Help Center:
- Verify all documents are clear, legible, and complete.
- Use blue or black ink on all documents.
- Verify each paper submitted contains the student’s name and NSHE ID.
- Verify that the IRS taxes are signed.
- If a signature is required, the signature must be hand-signed.

Unusual Enrollment History Appeal Form
Complete this form if you already submitted the Unusual Enrollment History Form, and the Office of Financial Aid & Scholarships has determined that you received aid at a previous institution but did not earn any credits in a semester when you received federal aid. This appeal process provides you the opportunity to explain why you didn't earn any credits during that semester. To retrieve a copy of the Unusual Enrolment History Appeal Form submit a request to the Self-Service Help Center.

Federal Loan Change Form
Complete this form if you are requesting additional student loan eligibility, previously declined your financial aid, canceled previously accepted financial aid, or changed your housing arrangements (On-Campus, Off-Campus, With Parents).
Federal Loan Proration Form
Complete this form only if you are an undergraduate student and will be enrolled in one semester of the academic year (Fall, Spring, or Summer).
Loan Discharge Reaffirmation Form
Complete this form if you previously had a student loan discharged because of a total permanent disability and you want to borrow additional federal student loans.
One Semester Loan Adjustment Form
Complete this form if you would like your federal student loans adjusted to one semester up to maximum borrowing eligibility.

Parent PLUS Loan Replacement Form
Complete this form if the parent PLUS loan was denied and the student desires to borrow additional funds under the unsubsidized loan program. Graduate students are not eligible to complete this form.

Ability to Benefit Form
Complete this form if you are required to demonstrate your ability to benefit from federal student aid programs. Your Student Aid Report (SAR) indicates if this is required. You can verify your ability to benefit from federal student aid programs by documenting your high school completion status.
Alternate Need Determination Form
Complete this form by logging onto Self-Service to determine your financial need for select UNLV scholarship and grant programs as a non-FAFSA eligible student. Do not complete this form if you are a U.S. Citizen or an eligible non-citizen.
Citizenship Affidavit
Complete this form if you filled out your FAFSA application, and the U.S. Department of Education could not confirm your citizenship status. This form is to be completed if you cannot present the required citizenship documents to the UNLV Financial Aid & Scholarships Office. Note: Until further notice, the notary section of the form is waived due to the COVID-19 pandemic.

Unusual Enrollment History
Complete this form if the United States Department of Education has determined you have attended multiple schools as a federal financial aid recipient.
Scholarship Forms
Millennium Scholarship Co-Enrollment Form
Complete this form if you are a Millennium Scholarship student and you wish to take a portion of your credits at another Nevada System of Higher Education (NSHE) college or university.
Millennium Scholarship Study Abroad Partnership Form
Complete this form if you are participating in a transfer credit (partnership) program to request matching grant funds in place of the Millennium Scholarship.
Millennium Scholarship Pass/Fail Grades
Suppose you lose eligibility for the Millennium Scholarship due to your GPA at UNLV but received a “Satisfactory” grade for a course. In that case, you can appeal the scholarship cancellation by turning in the Millennium Scholarship Recorded Grade Form. On this form, your instructor can indicate a letter grade, which, if included in your GPA calculation, would make you eligible for the Millennium Scholarship. This grade will not be a part of your official UNLV GPA and is used for Millennium Scholarship purposes only. The Financial Aid & Scholarships Office will calculate an unofficial semester GPA using the reported grade and submit a request to the state Millennium Scholarship office for final approval.
You must pay all charges on time, and no late fees will be waived for students waiting on the decision of their appeal.
Millennium Summer Reimbursement Form
Complete this form if you are a Millennium Scholarship student, but you are not degree-seeking at the eligible institution you are enrolling in summer classes.
Millennium Students with Documented Disabilities Form
Complete this form if you are a student with a documented physical or mental disability or who were previously subject to an individualized education program and are requesting to enroll with Millennium Scholarship support in fewer than the minimum semester credit hours or an extension of the expiration date for funding.
Summer Term Forms
Complete this form if you are enrolling for the summer 2023 term and would like to be considered for financial aid for the summer term by the Office of Financial Aid & Scholarships.

Asset Worksheet

IRA/Pension Rollover Worksheet
