Submit Immunization Records

Requirements

Nevada Administrative Code (441A.755) requires that all students, who attend a university, must provide documentation of having received the following immunizations unless excused by a medical condition or religious belief with the appropriate exemption form on file with the Office of the Registrar:

  1. Two doses of the measles, mumps, and rubella (MMR). The first dose must be on or after your first birthday.
  2. One dose for tetanus/diphtheria (TD), taken in the last 10 years
  3. All incoming or enrolled freshmen (regardless if they are living on campus or not) under the age of 23 at the time of initial enrollment are required to submit proof of having received the Meningitis vaccination with at least one dose occurring on or after age 16. 

Acceptable versions of the Meningitis vaccination are:

  • Menactra or Menveo (MenACYW-135)
  • MCV4

Versions of the Meningitis which will not meet this requirement are:

  • Bexero or Trumemba (MENG B)
  • HIB Meningitis

Acceptable Proof of Immunization

  • Childhood immunization card signed by a physician or nurse, including the facility name and address
  • High school or college records
  • Military, state, or county records
  • Lab report that indicates immunity (titer test for MMR)
  • Health department records
  • Records from a pharmacy, including name and address
  • Records from the UNLV Student Health Center
  • Records from WebIZ (for immunizations received in Nevada only)

The State Board of Health Vaccine Exemption Forms are available for download online on the Nevada System of Higher Education (NSHE) forms website.

Please submit your immunization exemption forms to the Rebel Success Hub. Select the immunizations that you are exempting from.

Where to Get Immunizations

Please upload your immunization records to the Rebel Success Hub.

  • Students who received immunizations in Nevada may access their immunization records from WebIZ.
  • Students who received immunizations at the UNLV Student Health Center may download their immunization records by visiting the patient portal, UNLV WellnessView. If you have any difficulty downloading your records, please email a request to shc@unlv.edu.

  • Step 1: Log into MyUNLV with your NSHE ID
  • Step 2: Click on the Tasks tile and navigate to your To-Do List to review the details and instructions for submitting your missing immunization records 
  • Step 3: Upload proof of immunization to the Rebel Success Hub
    • The hold on your account will be removed once your records have been received and processed by the Office of the Registrar.

  • If you intend to enroll in online classes only, log into MyUNLV and complete the Online Class Enrollment Agreement. After accepting the terms, the Office of the Registrar will resolve the enrollment hold to allow you to register for online classes.

  • All of the required immunizations are available on campus at the UNLV Student Health Center:
    • COVID-19: no charge
    • Tetanus/Diphtheria (Td): $46.00/dose
    • Measles, Mumps, and Rubella (MMR): $98.00/dose
    • Meningitis: $125.00/dose
  • Cash, credit card, and debit card payments are accepted. Immunizations are available while supplies last. Prices are subject to change. 
  • Immunizations are available by appointment only. Appointments can be made by calling

  • You will be notified in the Rebel Success Hub that your immunization records have been processed. The immunization will also be removed from your MyUNLV To Do List.
