  1. Create a form using Macromedia Dreamweaver or similar web authoring application.
  2. Find the opening <form> tag and replace it with the following lines of code. Be sure to replace the text with your own information as indicated below.

<form action="/ServerBin/E-Mail" method="post">
<input name="recipient" value="Insert your email address here. Multiple addresses may also be used; just separate them with a comma (no space)" type="hidden" />
<input name="subject" value="Insert the text you wish to use for the email subject line here" type="hidden" />
<input name="return_message" value="Insert the message you wish to have appear on the webpage that is returned to the client when he/she clicks the submit button." type="hidden" />
  1. Publish your form to the server.
  2. Open your web browser and test your form. You should see a thank you page after clicking the "Submit" button.
