Stop the Spread of COVID-19
Individuals who have been on a UNLV campus and test positive for COVID-19 should complete the voluntary online reporting form. This information assists UNLV and the Southern Nevada Health District in notifying others of the need to self-quarantine and to seek testing. Cases are posted without identifying information.

About UNLV’s Process
- Once the case notification form is submitted to UNLV, the UNLV contact tracing team assisting the Southern Nevada Health District (SNHD) is automatically notified of the report.
- If the case has not been previously investigated by SNHD, a full investigation is conducted to identify risk factors and contacts with possible exposure.
- The identified contacts are notified by a team of SNHD contact tracers of their possible exposure and given specific direction, which may include self-quarantine and testing.
- Information gathered throughout the process is confidential and information reported by the university is in aggregate without personally identifiable information.
- The person filling out the form may also request to be contacted by UNLV Human Resources, the Student Health Center, the Faculty and Staff Treatment (FAST) Center, or the Provost's Office for assistance related to coursework.
About Clery Act Notifications
UNLV will notify students, faculty, and staff as required by the Clery Act of the presence of COVID-19 on campus. This will not occur for every case on campus per the act as it is limited to the first identified case of the semester (on each UNLV property) and for exposures in a group setting where all potential close contacts cannot be individually identified.
