- Assessment Plans: Plans are renewed every three years and should be submitted via email to assessment@unlv.edu. Coversheets with instructions can be found on the forms page.
- Data/Reports: You may request data/reports from the surveys distributed by the Office of Academic Assessment: Graduating Senior Exit Survey & First Year Seminar Learning Outcomes Survey. If you are in need of survey data, please complete the Data Request Form.
