  If your admission to UNLV has been canceled, and you would like to re-enroll, complete this form and submit it to the Office of Admissions. You may need to submit additional documentation with this form including official transcripts.

• Nondegree-Seeking Undergraduate Student Application
  Use this form if you have never attended UNLV or attended prior to summer of 1994 and you wish to apply as a nondegree-seeking student.
