Appeal Forms

Dependency Status Appeal

Complete this form if you want to be considered an independent student for financial aid purposes. Most students will not be approved. Please read the dependent/independent section of our website first before you decide to appeal and consult a financial aid counselor.

Satisfactory Academic Progress (SAP) Appeal

Complete this form if you have lost your eligibility to receive federal financial aid. Please read the SAP policy of our website first before you decide to appeal.

Special Circumstances Appeal

Complete this form if you or your family has experienced a significant loss of income. Some appeals may not be approved. Please read the special consideration section of our website first before you decide to appeal and consult a financial aid counselor.

Cost of Attendance Appeal

Complete this form if your total expenses for the academic year exceed the standard costs. Cost of Attendance Appeal forms are accepted after the third week of Fall classes for each academic year.

Scholarship Probation or Cancellation Appeals

Complete this form if you would like to appeal the cancellation of a scholarship or appeal a scholarship's probation status. Reviews may take up to 30 days and are addressed in the order they are received. Appeals will not be approved to change scholarship award amounts. Requests for new scholarships received via the appeal form will not be reviewed.

Selective Service Failure to Register Appeal

Consider completing this form if you failed to register for the Selective Service program according to federal law.

Certification Forms

Consortium Agreement Form

Complete this form if you would like to combine your credit hour enrollment between two schools. Not all students need to complete this form. Please read the information on consortium agreements first before you submit this form and consult a financial aid counselor.

  • Fall 2013 Consortium Agreement with the College of Southern Nevada (CSN): Must submit be received by August 30, 2013
  • Fall 2013 Consortium Agreement — Complete this form if you are attending any school (other than CSN) and wish to combine your financial aid between UNLV and your other school: Must be received by August 30, 2013.

Loan Discharge Reaffirmation Form 2013-2014

Complete this form if the office needs to clarify you had federal student loans previously discharged because of a permanent disability.

Parent FAFSA Refusal Form

Complete this form to report you are unable to provide income information for your parent or if your parents refuse to complete the parental section on the FAFSA.

Teacher Certification/Licensure Form

Complete this form if you are in a teacher certification or licensure program and the office needs to verify if you are in an eligible program to receive federal financial aid.

Unusual Enrollment History Form

Complete this form if the U.S. Department of Education has indicated you have attended multiple colleges/universities over several years.

Private Student Loan Form

Complete this form if you are applying for a private student loan or you would like to make changes to the loan amount of a previously approved private student loan.

Check Return Form

Complete this form only if you are returning a refund check to UNLV Financial Aid & Scholarships.

Loan Credit Proration Form

Complete this form only if you are an undergraduate student and will be enrolled in only one semester of the academic year (Fall, Spring or Summer).

Graduate Student PLUS Loan Application Form

Complete this form if you are a graduate student and want to borrow a PLUS loan for yourself.

Parent PLUS Loan Application Form

Complete this form if you are a parent and want to borrow a PLUS loan for your student.

Parent PLUS Loan Substitution Form

Complete this form if the parent PLUS loan was denied and the student desires to borrow additional funds under the unsubsidized loan program. Graduate students are not eligible to complete this form.

One Semester Loan Adjustment Form

Complete this form if you would like your federal student loans adjusted to one semester up to maximum borrowing eligibility.

Complete this form only if you are granting a third party to access limited information about your financial aid record.

Enrollment Change Form

Only complete this form if the credit hours your financial aid is based upon will differ from your actual credit hours of enrollment.

Revision Request Form

Complete this form if you previously declined your financial aid, you are requesting additional student loan eligibility, you are cancelling previously accepted financial aid, or changing your housing arrangements (On campus, off campus, with parent).

Scholarship Forms

Millennium Scholarship Co-Enrollment Form

Complete this form if you are a Millennium Scholarship student and you wish to take a portion of your credits at another Nevada System of Higher Education (NSHE) college or university.

Millennium Scholarship Study Abroad Partnership Form

Complete this form if you are participating in a transfer credit (partnership) program to request matching grant funds in place of the Millennium Scholarship.

Millennium Scholarship Pass/Fail Grades

If you lose eligibility for the Millennium Scholarship due to your GPA at UNLV but received a “Satisfactory” grade for a course, you can appeal the scholarship cancellation by turning in the Millennium Scholarship Recorded Grade Form. On this form, your instructor can indicate a letter grade which, if included in your GPA calculation, would make you eligible for the Millennium Scholarship. This grade will not be a part of your official UNLV GPA and is used for Millennium Scholarship purposes only. The Financial Aid and Scholarships office will calculate an unofficial semester GPA using the reported grade and submit a request to the state Millennium Scholarship office for final approval.

Please note, the process can take several weeks to complete, and you should not assume that your request will be approved. You must pay all charges on time; no late fees will be waived for students waiting on the decision of their appeal.

Financial Aid & Scholarships Office Scholarship Application

To be considered for most UNLV scholarhips, you do not need to submit any application materials. To determine if scholarships for which you may qualify require you to submit anything, please use the UNLV Scholarship Database. Any requirements will be listed in the Scholarship Application Information section on the page for each scholarship within the database.
