Appeal Forms

Satisfactory Academic Progress (SAP) Appeal

Complete this form if you have lost your eligibility to receive federal financial aid. Please read the SAP policy of our website first before you decide to appeal.

Scholarship Probation or Cancellation Appeals

Complete this form if you would like to appeal the cancellation of a scholarship or appeal a scholarship's probation status. Reviews may take up to 30 days and are addressed in the order they are received. Appeals will not be approved to change scholarship award amounts. Requests for new scholarships received via the appeal form will not be reviewed.

Selective Service Failure to Register Appeal

Consider completing this form if you failed to register for the Selective Service program according to federal law.

Special Consideration Appeals

If you or your family experienced a significant change in your financial situation, the Financial Aid & Scholarships Office, under limited circumstances, may be able to re-evaluate your eligibility for financial aid for the school year. Contact us to discuss your personal extenuating circumstances with a financial aid counselor. A financial aid counselor will instruct you of the appeals process according to your circumstances.

Loan Forms

Private Student Loan Form

Complete this form if you are applying for a private student loan or you would like to make changes to the loan amount of a previously approved private student loan.

Check Return Form

Complete this form only if you are returning a refund check to UNLV Financial Aid & Scholarships.

Loan Credit Proration Form

Complete this form only if you are an undergraduate student and will be enrolled in only one semester of the academic year (Fall, Spring or Summer).

Parent PLUS Loan Substitution Form

Complete this form if the parent PLUS loan was denied and the student desires to borrow additional funds under the unsubsidized loan program. Graduate students are not eligible to complete this form.

One Semester Loan Adjustment Form

Complete this form if you would like your federal student loans adjusted to one semester up to maximum borrowing eligibility.

Health Professions Student Loan Application

Complete this application if you are a Doctor of Dental Medicine (DMD) student who would like to be considered for the Health Professions Student Loan program.

Complete this form if you are required to demonstrate your ability to benefit for federal student aid programs.

Loan Discharge Reaffirmation Form

Complete this form if you previously had a student loan discharged because of a total permanent disability and you want to borrow additional federal student loans.

Revision Request Form

Complete this form if you previously declined your financial aid, you are requesting additional student loan eligibility, you are cancelling previously accepted financial aid, or changing your housing arrangements (On campus, off campus, with parent).

Scholarship Forms

Millennium Scholarship Co-Enrollment Form

Complete this form if you are a Millennium Scholarship student and you wish to take a portion of your credits at another Nevada System of Higher Education (NSHE) college or university.

Millennium Scholarship Study Abroad Partnership Form

Complete this form if you are participating in a transfer credit (partnership) program to request matching grant funds in place of the Millennium Scholarship.

Millennium Scholarship Pass/Fail Grades

If you lose eligibility for the Millennium Scholarship due to your GPA at UNLV but received a “Satisfactory” grade for a course, you can appeal the scholarship cancellation by turning in the Millennium Scholarship Recorded Grade Form. On this form, your instructor can indicate a letter grade which, if included in your GPA calculation, would make you eligible for the Millennium Scholarship. This grade will not be a part of your official UNLV GPA and is used for Millennium Scholarship purposes only. The Financial Aid and Scholarships office will calculate an unofficial semester GPA using the reported grade and submit a request to the state Millennium Scholarship office for final approval.

Please note, the process can take several weeks to complete, and you should not assume that your request will be approved. You must pay all charges on time; no late fees will be waived for students waiting on the decision of their appeal.

Financial Aid & Scholarships Office Scholarship Application

To be considered for most UNLV scholarships, you do not need to submit any application materials. To determine if scholarships for which you may qualify require you to submit anything, please use the UNLV Scholarship Database. Any requirements will be listed in the Scholarship Application Information section on the page for each scholarship within the database.

Options for Providing IRS Income Tax Data

If selected for verification and you/your parent(s) filed an income tax return for 2013, you are required to provide income tax data as part of the financial aid application process. This document provides instructions regarding the options you may use to provide the required 2013 income tax data to the Financial Aid & Scholarships Office.

Verification Worksheet for Dependent and Independent Students

Complete the correct worksheet as described to you within your MyUNLV Communication Center message from the Financial Aid & Scholarships Office.
