Confidential Financial Certification Form

Please complete the Confidential Financial Certification form. Include a bank letter showing the amount listed on the Confidential Financial Certification form and mail or fax the forms to the Office of International Students & Scholars.

NOTE: The certification form has to be signed by the guarantor (owner of money named in the bank letter).
