Acquiring Data and/or Services

The Center for Health Information Analysis (CHIA) provides UB acute care Hospital Inpatient data for research and analysis. Data release is only available for valid analysis. Validity will be determined at time of purchase.

TO PURCHASE DATA FOLLOW THE NEXT THREE STEPS:

1) Mail a signed Limited Data Set Use Agreement to the Nevada State HIPAA Officer

The signing of the Use Agreement is normally a one time process and need not be signed again for future purchases.

2) Fill out a Data Request Form. Prices can be found on the form.

3) Make a check payable to BOARD OF REGENTS . Please do not make check payable to Center for Health Information Analysis. Mail the check to our Center:
