Health Insurance Open Enrollment May 1-30

The State Health plan has mailed out the Open Enrollment Packets. If you do not receive your packets by May 5, call the Public Employees Benefits Program (PEBP) at 1-800-326-5496.

If you choose the same plan with the same insured members, no action is required. Premium changes will be effective with the August payroll.

Open Enrollment — May 1-31
An Open Enrollment meeting will be held on the UNLV campus on May 22 at 1 p.m. at the Student Union, Room 208. Representatives from PEBP will be available to answer questions. Additionally, PEBP and university vendors will be available from 9 a.m. to 3:30 p.m. in Rooms 207 and 208 at the Student Union.

Premium Holiday
The PEBP board approved using some of the plan's excess reserves for a one-time "premium holiday" for July 2008. This means that the premiums for all participants will be waived for the month of July. The new premium deductions for Plan Year 2009 will resume in August.

Health insurance premium rates are going up for most employees, especially those who cover dependents. Rate information is available on line at http://hr.unlv.edu/Benefits/py09activerates.pdf

Health Assessment
Participants and their covered spouses will again need to complete the Health Assessment Questionnaire (HAQ) to receive the additional benefit of cutting their PPO deductible in half and increasing their dental plan year maximum to $2,000 from $1,500. HMO participants and their covered spouses who participate in the HAQ will also receive the benefits of increasing their dental plan year maximum to $2,000 from $1,500. The deadline for completing the health assessment is May 31.

In order to receive the additional benefit listed above, both the employee and their covered spouse must complete the assessment. Participants 75 years and older are exempt and will automatically receive enhancements.

This year, participants have from April 1 to May 31 to complete the HAQ, giving participants an additional 30 days to complete the HAQ compared to previous years. Participants can complete the HAQ online by going to PEBP's website at pebp.state.nv.us or requesting a paper form by calling APS Healthcare at 888-323-1461 and selecting option 3. Paper forms are available for request from April 1 to May 5. All paper forms must be postmarked by May 31 to receive the incentive.

MAKING CHANGES
Online enrollment is NOT available this year. If you are making a change, you will need to complete an open enrollment form. Original forms are required by the plan. Faxed copies will not be accepted. If you choose the same plan with the same insured members, no action is required. Premium changes will be effective with the August payroll.

Open Enrollment forms will need to be received in the UNLV Human Resources office in CSB, Room 237 (Mailstop 1026) by 5 p.m. on Friday, May 30.

If adding dependents, submit supporting documents, e.g. copies of birth certificates, marriage certificate, etc.

Benefit Counselors will be available in CSB, Room 237 to assist employees in making changes — no appointments necessary.
